Job Description
A great new opportunity exists for a Site Manager at our Respite program. Under the direction of the ACCS / Respite Program Director, this position is responsible for the management of a respite site which includes :
- Higher acuity of clients coming out of acute inpatient settings or State hospitals with little to no periods of stability
- On a near daily basis receiving clinical packets from DMH for both outreach and site based clients, reviewing clinical records for any active risk or safety concerns, determining the need for further review by ACCS PD
- Near daily coordination of care with HLOCs and at times outreach programs to arrange for admission, and ensure MAP regulations are being followed
- Involved in near daily MAP medsoft management changes related to discontinued, and new medications and coordination with Respite prescriber re : guidance and support
- Involved in daily engagement with all clients to assess level of risk, need, and connection to services / providers
- Writing and supporting caseworkers in writing ACA and IAP for all clients within a short time span; reviewing paperwork completed by caseworkers and auditing to ensure timely completion, adequate information provided, etc.
- Involved in opening avatar episodes for each admission and closing upon discharge. Adding additional practitioners for each client and each team member at respite.
- Involved in managing the mobile outreach census (GLEs don't have outreach clients)
- Weekly TEAMS mtg with DMH- the capacity to manage politics, be articulate, knowledgeable, and appropriate in all interactions with our contract holders.
- In addition to running weekly mtg with DMH, they must be able to collaborate with work with contract monitor as needed throughout the week (often a few times a week)
- A monthly collection of SDR data for each site and mobile client, submission of the monthly spreadsheet to the billing office internally
- Several times weekly communication with DMH Admin re : authorization numbers for admissions to program, discharge dates, and disposition data
- Community outreach with mobile clients
- supervision of multi-disciplinary staff, not just RE / RC as in GLE. They are co-supervising an RN, independently supervising mobile caseworkers, MHWs, and peer specialist
