Therapeutic Mentoring (TM) is a home and community-based service of structured, one-to-one mentoring sessions between a Therapeutic Mentor and a youth, designed to support a specific goal on the youth’s behavioral health treatment plan developed by a hub service (outpatient therapist, in home therapist or intensive care coordinator). This includes supporting, coaching, and training the youth in age appropriate behaviors; interpersonal communication; problem-solving and conflict resolution; relating appropriately to other children and adolescents, as well as adults, in recreational and social activities. Completes all clinical documentation and service plans on-line using AVATAR Electronic Health Record System.
Responsibilities:
- Travel to youth’s home and throughout the community
- Engage youth in home, school and/or community activities to teach new strategies and skills that are linked to the youth’s treatment plan
- Support the youth in recognizing individual skills and strengths
- Help identify a natural support system and foster independence
- Regular coordination with collaterals providers, including schools, state agencies, outpatient therapists, in home therapists, intensive care coordinators and other team members
- Maintain accurate clinical charts and follow all clinical documentation timelines and requirements
- Participate in weekly supervision and staff meetings
- Meet agency productivity requirements

Schedule Monday through Friday 11am-7pm.
Office is based out of Marshfield. The Plymouth area may cover Marshfield, Duxbury, Plymouth, Middleboro, Scituate and Pembroke.
Required Skills
- A bachelor’s degree in a human services field and one year experience working with children/adolescents/transition-age youth or
- An associate’s degree in a human services field and one year experience working with children/adolescents/transition-age youth or
- A high-school diploma or GED and a minimum of two years’ experience working with children/adolescents/transition-age youth.
Required Experience
- Minimum of one to two years’ experience working with children/adolescents/transition-age youth depending upon education
- Must be a minimum of 21 years old
- Good communication skills (both written and oral)
- Must demonstrate the following characteristics: flexible, team player, non-judgmental, ability to grow and learn in the position, ability to provide support and quality care for the well being of clients and families served.
- Experience utilizing Electronic Health Record preferred
- Computer proficiency and ability to learn new systems and applications
- Valid MA State driver’s license, safe driving record and access to personal vehicle for client transportation
- CPR/First Aid Certification, may be obtained after hired
