What are the responsibilities and job description for the LVN Weekend On Call position at Aspire Hospice?
Duties:
- Provide direct patient care in various settings, including homes, ALF, PCH, and long-term care facilities
- Assess patient needs and develop individualized care plans
- Assess and monitor medications and treatments as prescribed by healthcare providers
- Monitor patient vital signs and report any changes or concerns to the appropriate medical personnel
- Educate patients and their families on Hospice and End of life care and needs
- Collaborate with other healthcare professionals to ensure comprehensive and coordinated patient care
- Document patient information accurately and timely
Experience:
- Minimum of 2 years of experience as a licensed vocational nurse.
- Experience in standards of nursing practice, including infection control, safety protocols, and documentation
- Familiarity with hospice care principles and practices
- Proficiency in using electronic medical record systems (e.g., eClinicalWorks, Cerner)
- Previous experience in hospice or palliative care
- Ability to lift and move patients when necessary
