Aspire Indiana Health is a nonprofit provider of comprehensive “whole health” services including primary medical care, behavioral health, recovery services and programs addressing the social determinants of health such as housing and employment. Aspire has health centers in five Central Indiana counties serving Hoosiers of all ages and walks of life.
Position Summary
The Supportive Services for Veterans and Families (SSVF) Case Manager will have certain direct responsibilities key to the success of the InteCare SSVF Program to ensure that Veterans and their families obtain and maintain permanent housing, as well as needed coordination and linkage to supportive services. The SSVF Case Manager is responsible for working with the Veteran and their families in order to locate, obtain and maintain permanent housing. In addition, the Case Manager will work with the Veteran, their family and the Peer Mentor as a team in order to determine and coordinate additional supports as needed.
Education/Experience/Requirements
- Bachelor's degree in social work, health care, or related field preferred from a college/university accredited by the US Department of Education.
- Relevant military experience can be substituted for education and/or case management experience
- Veterans with professional experience working with individuals and peers and/or professional experience working with Veterans preferred
- Must possess/maintain a valid driver's license, current automobile insurance and a driving record that meets the guidelines and requirements of the organization. Must have the ability to use private car to transport self
