This individual will oversee social service functions, including assessments, discharge planning, care coordination, and support for residents and their families. They will also serve as a liaison between residents, families, and interdisciplinary teams.
Key Responsibilities:
Conduct initial and ongoing psychosocial assessments
Develop and implement individualized care plans
Coordinate discharge planning and community referrals
Advocate for residents’ rights and well-being
Provide counseling and crisis intervention as needed
Collaborate with healthcare teams to ensure quality care
