Description
Provides support, instruction, and assistance to individuals as they work to increase their employment skills, social skills, explore their communities, develop life skills and contribute to/be involved in their communities in meaningful ways.
Requirements
1. Promotes, encourages, and supports the creation or opportunities for people with disabilities to Develop community relationships that connections can be made between the persons supported and community organization (volunteer, or join a club as example)
2. Protects and safeguards all confidential information about its clients, propriety information on ASPIRO and its operations. Confidential information should only be shared on a 'need to know basis' to perform their job.
3. Make decisions and work independently.
4. Assists with transportation needs of clients including providing supervision and assistance for clients departing and boarding buses and other vehicles, driving ASPIRO vehicles, which includes operation of wheelchair lifts.
5. Follows safe lifting practices of clients to and from wheelchairs or from reclining positions to sitting and/or standing. Operates automated lift equipment and utilize gait belts as needed.
6. Build strong relationships with the individuals that you work with to assist in learning and obtaining new skills that support continued movement toward more independence.
7. Communicate effectively in person and for record keeping.
8. Work cooperatively with all people.
9. Document case notes.
10. Implement Positive Personal Profiles and Skill Assessments to document goals and outcomes.
11. Promotes, encourages and supports Person Centered Planning. Works with staff to assist people receiving services, their families, guardians and significant others to organize and communicate what are important to them. Promotes, encourages and supports the development and evaluation of individualized goals based on a person’s strengths, preferences and needs.
12. Provides direct care, instruction, support, and supervision to people receiving services, as needed.
