Aspirus At Home in Ironwood, Michigan is seeking a SOCIAL WORKER to join our field team!
The Social Worker assists patients and families in coping with problems resulting from severe or long-term illness, and with difficulties in recovery and rehabilitation. Works with patients and families to obtain psychosocial history, assists patients in resolving environmental difficulties interfering with attainment of maximum benefits from medical care, and conducts routine discharge planning. Directs patients to designated community agencies or resources, as necessary. The Social Worker is a core interdisciplinary team member who, as a professional social worker, provides necessary social work services to patients / families under the direction of a physician.
HOURS : Part Time; 0.6 FTE or 48 hours per pay period. Day Shift.
Experience / Qualifications
- Possesses Michigan Certified Social Worker licensure or provisional license.
- One year medical social work experience preferred to gain full knowledge of federal, state and local regulations related to discharge planning and knowledge of health and welfare resources and facilities.
- MSW required.
- Possesses a valid Driver's License required.
- Possesses automobile liability insurance with minimum $100,000 / $300,000 coverage required, or $300,000 combined single-limit liability.
- Has an acceptable driving record.
