Adult Case Manager
Job Summary:
The Adult Case Manager provides services that focus on the specific needs of adults with behavioral health diagnoses and decreased ability to function independently in their homes and in the community.
Key Responsibilities:
- Develop client-centered individual service plans to ensure approved services through the Administrative Service Organization (ASO) Managed Care System.
- Maintain current benefit and financial information for clients with cognitive disabilities to ensure benefits continue to be applicable.
Work Environment:
This position requires a regular office schedule, working five days per week, Monday-Friday, between the hours of 6am and 5pm. Travel is required in your designated area.
Qualifications:
- High school diploma or equivalent.
- Proof of vehicle insurance at $50K/$100K and a valid Maine driver's license are required.
- MHRT/C or MHRT/CP certification.
- Intermediate level computer experience.
