About the Role
">We are seeking a Client Advocacy Professional to join our team at Assistance Plus!
In this role, you will provide support to adults with behavioral health diagnoses and decreased ability to function independently.
Your responsibilities will include developing individual service plans, ensuring benefits continue to be applicable, and maintaining current benefit and financial information for clients with cognitive disabilities.
This position requires strong advocacy skills, as well as excellent communication and interpersonal skills.
Key Responsibilities
- Develop client-centered individual service plans
- Maintain current benefit and financial information for clients with cognitive disabilities
- Ensure benefits continue to be applicable
- Work collaboratively with clients, families, and other stakeholders to achieve desired outcomes
Requirements
- Bachelor's degree in a related field or equivalent experience
- MHRT/C or MHRT/CP certification
- Intermediate level computer experience
- Proof of vehicle insurance and valid driver's license required
- Ability to travel in designated area
