The impact you will make:
- Assist patient with personal hygiene, elimination, / nutritional support, ambulation and exercise according to the patient’s needs and as specified on the CHHA Assignment.
- Ensure quality and safe delivery of CHHA services per written plan of care.
- Communicate with RN, Rehab Case Manager or Clinical Supervisor on a regular basis regarding changes in the patient’s condition and/or needs.
Who you are:
- Completion of an approved training program pursuant to Federal/State law.
- Current, active California CHHA certificate with criminal background check.
- Current CPR, health clearance and completed skills assessment.
