What You’ll Do:
- Deliver hands-on care that includes medication administration, client education about prescribed medications, and laboratory processing with precision and empathy.
- Work closely with our medical treatment team to ensure medications are ordered, received, transcribed, and administered as prescribed.
- Champion client care by monitoring safety, identifying quality improvement opportunities, and ensuring compliance with the highest standards of care.
SCHEDULE INFORMATION:
- 8-hour shifts: 7am-3:30pm
- 5 shifts per week (40 hours total)
- Some weekends & rotating holiday availability required
MAJOR DUTIES AND RESPONSIBILITIES:
- Adherence to facility policy and procedures, as well as State and Federal laws.
- Consult with the physician regarding patient medications and treatment.
- Participate in diagnostic test orders to help providers identify and assess the patient’s clinical problems and health care needs.
- Comply with medical records policies regarding the complete and accurate documentation of patient care. Initiates and maintains required records and legal documents.
- Educates patients about health and illness’ and promotes health maintenance
- Collaborates with the clinical team regarding the patient’s treatment, health and illness prevention.
- Maintain and review patient records, charts and other pertinent information.
- Administers medications, coo vaccinations, and renders treatment to clients.
- Communicates with providers, RN’s, LPN’s, and medical professionals ensuring a continuum of care for each client.
- Oversee and maintain the tracking of medical supplies and inventory.
- Observe and record client behavior as needed or directed.
- Conduct nurse-to-nurse communications with hospitals in the event a client is sent out, ensuring all discharge paperwork is received upon arrival back to the facility.
- Maintain reports of patients’ medical histories, and monitoring changes in their condition.
- Other duties as assigned.
PHYSICAL REQUIREMENTS:
- Ability to sit and stand to retrieve medication.
QUALIFICATIONS:
- Valid LPN license in the state of New York.
- 3-5 Years of nursing experience, preferably in a long-term care environment.
- Ability to explain treatment and conditions to clients within the age range of 50-90.
- Working knowledge of healthcare laws and regulations.
