A Family Liaison and Community Educator is needed immediately for a growing home care business in MetroWest & Boston, MA area.

As the Family Liaison and Community Educator, you will:
- Lead brand awareness/education efforts, and prepare/deliver presentations to groups in the community
- Conduct training sessions for professionals and families that focus on dementia care, Parkinson’s disease care and more
- Identify and maintain a database of influence centers (hospitals, nursing homes, assisted living with rehabilitation or respite care, doctor's offices and other health providers) - contacts who can refer leads/potential clients
- Personally visit and arrange meetings with contacts - this includes private, public, and non-profit organizations.
- Perform new client assessments; create and input care plans, and coordinate with the office staff to ensure client needs are communicated and care starts in a timely manner.
- Manage all leads - responding to and following up on all lead inquiries, and onboarding new clients/families
- Achieve visit frequency with key targets and deliver messaging aligned with our marketing program. Track results and meet weekly with manager on progress.
- Contribute to strategic plan for long-term business growth.
Full-time hours. Flexible schedule with some hybrid work offered. Approx. 60-80% time working in the community.
Requirements/Qualifications:
- LMSW, LBSW, preferred with experience in the senior care industry
- Occupational Therapist, Physical Therapist, LPN, RN considered
- Experience with the elderly preferred; Passion for seniors a must
- Exceptional presentation skills and highly organized
- Excellent verbal and written communication, and multi-tasking skills
- Sufficient computer skills to foster a paperless work environment
- Travel to clients' homes and community partners in the MetroWest and Boston areas
