Assisting Hands (Hudson) is looking for an experienced RN with a valid license in New Jersey and a minimum of 1 year of Home Care experience.
Your responsibilities include:
Assisting Hands (Hudson) is looking for an experienced RN with a valid license in New Jersey and a minimum of 1 year of Home Care experience.
Your responsibilities include:
- Conducting thorough client assessments and re-assessments
- Formulating comprehensive care plans
- Performing 30-day check-in and 60-day care monitoring visits
- Conducting supervisory visits
- Regular communication with patients, families, caregivers and care-stakeholders.
Qualifications:
- Valid nursing license in State of New Jersey
- One year experience in Home Care highly preferred.
- Has a valid driver's license and a reliable mode of transportation.
- Able to travel to clients’ homes throughout Hudson and Southeast Bergen counties.
- Strong communication and interpersonal skills
- Professional Liability Insurance 1M – 3M(Preferred)
