About the Role :

We are seeking a dedicated and compassionate Licensed Practical Nurse to join our team at Assisting Hands of South Jacksonville. As an LPN, you will play a crucial role in providing high-quality care to our clients and making a positive impact in their lives. We are seeking to provide world class non-skilled care to Clay and Duval

Responsibilities :

  • Assist with the development and implementation of care plans
  • Provide initial and followup assessments of ADL's and iADL's
  • Collaborate with the care team to ensure the best possible outcomes for our clients
  • Improve care by providing routine quality assessments of our home care population
  • Assist with marketing / networking as needed

    • Requirements :

  • Current LPN license in the state of Florida
  • Previous experience in Skilled Home Care preferred
  • Strong communication and interpersonal skills
  • Compassionate and patient-centered approach to care
  • Ability to work independently and as part of a team
