Role Description
This is a full-time remote role for an RN Care Coordinator. The RN Care Coordinator will be responsible for assessing patient needs, developing personalized care plans, coordinating with healthcare providers, monitoring patient progress, and advocating for patient well-being. Additionally, the RN Care Coordinator will ensure compliance with healthcare regulations and provide education and support to patients and their families.
Qualifications
- Registered Nurse (RN) license
- Experience in case management and patient assessment
