Home & Community Based Support Professional (Per diem)

Part-Time: 8-20 hrs/week

Join our LifeNet program as a Support Partner, offering independent living support for autistic adults. You’ll provide 1:1 support in clients' homes and communities with the backing of our collaborative team.

Your Role:

• Work with clients to implement their strengths-based LifeNet Support Plan in areas such as:

  • Home organization and housekeeping

  • Meal preparation, nutrition, and food safety

  • Healthcare appointments and medication reminders

  • Money management, social engagement, recreation, and exercise

  • Personal hygiene and self-care

• Track and report observations, time, mileage, and notes through our case management system

• Participate in virtual supervision meetings for support and collaboration

• Travel Requirements: Most client visits occur within the I-495/I-95 belts. Travel time is compensated, with mileage reimbursed at the federal rate.

Additional opportunities in the role:

• Organize or facilitate social/recreational activities

• Support event planning and research client resources

• Experience supporting autistic adults or related human services experience

• Valid driver’s license, clean driving record, and reliable vehicle

• Strong communication, time management, and organizational skills

• Proficiency with tech tools (Zoom, Google Suite, MS Word)

• Patience, adaptability, and ability to work independently and within a team

• Willingness to undergo state and federal background checks
