Case Manager Mental Health Professional Intake Bilingual

The Association for Individual Development (AID) is a non-profit organization whose mission is to empower people with physical, developmental, intellectual, mental health challenges; those who have suffered a trauma; and those at risk, to enjoy lives of dignity and purpose. We are looking for a Case Manager Mental Health Professional Intake Bilingual for our Behavioral Health department who demonstrates this mission and wants to work for an organization that makes a difference in the community.
Schedule: Monday-Friday 8:30am-5:00pm OR 4 days per week 8:00am-6:30pm (Mon-Thurs. or Tues-Friday)
What you will be doing?
- Complete Intake/IM CANS as assigned
- Provide/Arrange Outpatient therapy-individual, as assigned
- Provision of assessment, counseling, skills training and case management services to a caseload of clients who are diagnosed with persistent mental illness disorders. Services are provided in individual treatment settings.
- Conduct new client intakes including IM CANS and LOCUS Assessments as assigned.
- Provide documentation of services in accordance with DHS and CARF.
- Advocate for and maintain rights of clients that are served.
- Assignment of clients to case managers’ caseloads in coordination with program manager.
- This job position may have some work components that can be performed remotely. Remote work arrangements are not a right or entitlement of employment. They are discretionary and subject to demonstrated performance and operational needs. Approval may be rescinded at any time at the sole discretion of management.
Essential Job Responsibilities:
- Work scheduled hours, be flexible to meet client and program needs, including cross training and coverage in other programs as assigned by supervisor. Complete clinical assessments as assigned that meet the requirements of program funders and applicable administrative rules
- Complete all required documentation in a timely manner including updating the intake trackers and call logs daily
- Acquire and maintain required certifications and trainings.
- Employee promotes awareness and respect for the diversity of our clients, employees, families and other stakeholders.
- Schedule new client intakes within timeframe set by agency policy.
- Complete Consent for Assessment, IM CANS and LOCUS assessment in a timely fashion using Cx360.
- Meet minimum service hour provision productivity standards monthly

Education: Bachelor’s degree in a human services field (required)

Additional Job Responsibilities:
- Assist in transitioning clients into needed services
- Obtain input from consumers and other stakeholders on how to improve service provision
- Provide outcome evaluation data to supervisor
- Complete all mandated trainings
- Maintain a positive professional working relationship with agencies and other stakeholders
- Maintain a positive professional working relationship with coworkers within AID Assist clients in identifying needed BH services
- Participate in agency committees/meetings as assigned
