The Association for Individual Development (AID) is a non-profit organization whose mission is to empower people with physical, developmental, intellectual, mental health challenges; those who have suffered a trauma; and those at risk, to enjoy lives of dignity and purpose. We are looking for a QIDP – Case Manager – Home-Based Supports who demonstrates this mission and wants to work for an organization that makes a difference in the community.
Be able to work Monday - Friday from 9am-5pm.
What will you be doing?
The Home-Based Supports Case Manager is responsible for assisting assigned caseload of individuals and families by providing training to enable self-directed services. The Case Manager provides guidance to Home-Based Support program participants with managing the services and supports budget as well as recruiting Personal Support Workers. The Case Manager is responsible to assist with the implementation of the Personal Plan and develop Implementation Strategies where required. The Case Manager provides assessment and referral or connection to supports and services that maximize community living skills and community inclusion. The Case Manager may support individuals in Community Day Services programs by providing documentation and services in accordance with DHS, CARF, or DPH standards as applicable. AID facilitates services for individuals from diverse backgrounds, embraces differences in individuals and promotes a positive attitude with peers and community members. This job position may have some work components that can be performed remotely. Remote work arrangements are not a right or entitlement of employment. They are discretionary and subject to demonstrated performance and operational needs. Approval may be rescinded at any time at the sole discretion of management. May work remotely for up to 20% of the work week with approval from the I/DD Community Supports Manager.

Education:

Bachelor's Degree in Human Services.
License/certifications:
-
Previous QIDP training highly preferred.
Experience:
-
One year of experience working directly with individuals with intellectual and/or developmental disabilities.
Physical:
-
Climb stairs daily
-
No medical restrictions which would prohibit implementation of a lift/transfer or implementation of behavioral programs.

Push, pull, reach, stoop/bend over and use step stool daily.

Implement behavior support strategies and respond to emergency behavioral issues.
-
Requires significant walking and standing on a daily basis.
Equipment:

Computer and software including Microsoft Word, Outlook, Excel, and Teams; Zoom platform and application; Internet browser and Microsoft SharePoint platform.

Alarm Systems for monitoring fire, weather, security, etc.

Telephone and voicemail.

Office Equipment including printer/scanner, copy machine, calculator, shredder, etc.

Adaptive equipment as defined in individual strategies including hoyer lift and accessories, shower chairs or benches, walkers, wheelchairs, etc.

Agency vehicles with and without wheelchair lifts.
Salary : $56,160
