The Association for Individual Development (AID) is a non-profit organization whose mission is to empower people with physical, developmental, intellectual, mental health challenges; those who have suffered a trauma; and those at risk, to enjoy lives of dignity and purpose. We are looking for an Outreach Specialist Senior Services Case Manager Mental Health Professional who demonstrates this mission and wants to work for an organization that makes a difference in the community.
What you will be doing?
- In order to bridge gaps within the community, the Outreach Specialist Senior Services CM will provide outreach and engage with seniors in the community in order to provide case management, community support services, advocacy, and linkage to resources in Aurora Township.
- The Outreach Specialist Senior Services CM will engage with seniors in Aurora Township in order to:
- Reduce the risk of social isolation
- Complete needs assessment and link individuals to appropriate community resources
- Create a resource map for the aging community
- Address mental health and psychiatric concerns and assist with access to AID services as needed
- May require a small client case load in order to provide case management and counseling services to senior clients who are diagnosed with persistent mental illness
- This job position may have some work components that can be performed remotely. Remote work arrangements are not a right or entitlement of employment. They are discretionary and subject to demonstrated performance and operational needs. Approval may be rescinded at any time at the sole discretion of management.
Essential Job Responsibilities:
- Work scheduled hours and be flexible to meet client and program needs.
- Engage in community outreach to provide advocacy and support to seniors in the community.
- Knowledge of issues impacting older adults and ability to identify and access resources.
- Provide linkage to community resources and warm hand-offs to referrals when applicable.
- Work with partner agencies in the community to identify seniors in need of services.
- Manage caseload and provide effective counseling services to clients in variety of settings (in their home, community, office setting, or by telehealth).
- Help clients to obtain and maintain benefits (social security, medicaid, link card, etc.).
- Complete updates and reviews of clinical assessments that meet the requirements of program funders and applicable administrative rules by required deadlines.
- Complete required client documentation (assessments, encounters, consents) on a timely basis and maintain client records.
- Maintain and update Outlook calendar with daily details of employment activities.
- Meet minimum service hour provision productivity standards (MRO) monthly.
- Recognize and respond to emergency/crisis situations per agency/program procedures.
- Complete and maintain all mandatory trainings, assuring compliance with all agency, state and federal regulations.

Qualifications
What will you bring to the table?
Education: A Bachelor’s degree in a human services field is required. Master’s degree in human services preferred.
License/certifications: License preferred
Experience: Some experience working in social services required.
Skills: Fluent verbal and written Spanish and English preferred

- Basic computer skills, and the ability and willingness to utilize computerized programs including Microsoft Office and Cx360.

- Transport clients in personal vehicle when applicable.
- Participate in program team meetings and community meetings as assigned.
- Obtain input from consumers and other stakeholders on how to improve service provision.
- Provide outcome evaluation data to supervisor.

- Maintain a positive and professional relationship with co-workers and other agency staff.
- Participate in agency committees as assigned.
- Promote and maintain client rights and assure client choice.

- Participate in outreach events (i.e. resource fairs/events) to promote program services and expand network.
- Communicate with service delivery partners and other professionals, as needed, to facilitate the achievement of client’s wellness objectives.
