Overview
We are seeking a dedicated and compassionate Care Coordinator to join our team. The ideal candidate will play a crucial role in ensuring the highest quality of patient care by coordinating services and support for individuals in need. This position requires strong communication skills, a commitment to patient advocacy, and the ability to work collaboratively with healthcare professionals and families.
Chinese language skills would be very helpful 95% of the clients are of chinese background.
Duties
- Coordinate patient care services, ensuring that all aspects of care are delivered effectively and efficiently.
- Monitor patients' health status and provide ongoing assessments to identify changes in condition.
- Assist patients with Activities of Daily Living (ADLs) such as bathing, dressing, and mobility.
- Provide support for hospice care, ensuring comfort and dignity for patients in end-of-life situations.
- Implement care plans tailored to individual patient needs, including those with dementia or other cognitive impairments.
- Advocate for resident rights and ensure compliance with healthcare regulations.
- Conduct regular patient observations to track progress and report any concerns to the healthcare team.
- Provide first aid as necessary and respond promptly to any medical emergencies.
Requirements
- Previous experience in patient care or a related field is preferred.
- Knowledge of hospice care practices and dementia care strategies is highly desirable.
- Ability to lift patients safely and assist with mobility as needed.
- Strong observational skills to monitor patient conditions effectively.
- Excellent communication skills for interacting with patients, families, and healthcare professionals.
- Understanding of resident rights and commitment to advocating for patients' needs.
- Certification in first aid or CPR is a plus.
- A compassionate demeanor with a strong commitment to providing high-quality care.
