About the Role
The Afterhours Registered Nurse's primary focus is to provide comprehensive and timely delivery of hospice care for patients and their families after regular business hours, including nights and weekends.
- You will be responsible for triaging patient needs over the phone until a nurse can provide an in-person visit.
- You will respond appropriately and with urgency to crisis calls from patients and families after hours.
- You will make initial nursing evaluations to determine eligibility for hospice services within 48 hours of referral.
- You will perform after-hours visits, including admission visits, death visits, recertification visits, and routine visits.
- You will identify patient/family physical, psychosocial, emotional, and environmental needs, reassessing them as needed every 15 days.
- You will document problems, goals, interventions, and patient/family response to hospice care.
Requirements
To be successful in this role, you will need:
- One year of clinical experience.
In addition, you will need:
- Current RN licensure in the state of practice and one year of clinical experience.
- Current CPR Certification.
- Current driver's license, valid vehicle insurance, and access to a dependable vehicle or public transportation.
