Key Responsibilities:
- Conduct comprehensive and timely delivery of hospice care over the phone and in-person visits.
- Respond to crisis calls from patients and families with urgency and compassion.
- Maintain accurate documentation of patient needs, goals, interventions, and responses to care.
Requirements:
- One year of clinical experience as a registered nurse.
- Current RN licensure in the state of practice.
- CPR Certification and valid driver's license with access to reliable transportation.
