As an After-Hours Registered Nurse, you will play a vital role in providing comprehensive care to patients and their families after regular business hours. Your primary focus will be on conducting timely and thorough assessments, identifying patient needs, and developing effective care plans.

Responsibilities

• Triage patient needs over the phone until a nurse can provide an in-person visit.
• Respond to crisis calls from patients and families after hours with urgency and compassion.
• Makes initial nursing evaluations to determine eligibility for hospice services within 48 hours of referral if assigned.
• Perform after-hours visits, including admission visits, death visits, recertification visits, and routine visits.
