As an RN Case Manager, you will assume full nursing responsibility for delivering the Plan of Care for each hospice patient, continuously evaluating and improving your performance to increase productivity and quality of care.
Responsibilities
- Conduct initial nursing evaluations to determine eligibility for hospice services within 48 hours of referral.
- Identify patients' physical, psychosocial, and environmental needs, reassessing regularly (at least every 15 days).
- Develop and coordinate individualized plans of care, maintaining accurate clinical notes on patients' conditions.
- Document problems, goals, interventions, and patient/family responses to hospice care.
- Collaborate with patients, families, attending physicians, and IDG members to provide daily patient and family care.
Requirements
- One year of clinical experience.
- Current RN licensure in the state of practice, plus one year of clinical experience.
- Current CPR Certification.
- Valid driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
