We are seeking a compassionate and skilled Registered Nurse to join our team as a Registered Nurse. As a key member of our medical team, you will provide comprehensive nursing care to patients in their homes, working closely with physicians and other healthcare professionals to develop and implement personalized care plans.
Responsibilities
- Conduct thorough initial and ongoing assessments of patients' physical and emotional needs
- Develop and implement individualized care plans in collaboration with the patient's physician and other healthcare professionals
- Provide direct patient care, including wound management, medication administration, and education on disease management and self-care
- Collaborate with other healthcare professionals to ensure seamless transitions of care and effective communication
Requirements
- Current RN licensure in the state of practice
- Current CPR certification
- Valid driver's license, vehicle insurance, and access to a reliable vehicle or public transportation
