We are seeking a skilled Registered Nurse (RN) to join our team as a Home Health Clinician. As an RN with Assured Home Health, you will provide high-quality patient care in a Point of Care setting, working closely with other healthcare professionals to achieve optimal outcomes.
Key Responsibilities:
- Conduct comprehensive nursing evaluations and develop individualized plans of care in accordance with state laws and regulations.
- Provide clinical services within the scope of practice, including making initial and follow-up visits, evaluating patient needs, and submitting accurate documentation.
- Collaborate with physicians, family members, and community resources to ensure continuity of care and achieve patient goals.
Requirements:
- Current RN licensure in the state of practice.
- Current CPR certification.
- Valid driver's license and reliable transportation.
