We are seeking a skilled RN Patient Care Manager with experience in Home Health to join our team at Assured Home Health, part of LHC Group.

Key Responsibilities
- Supervise and coordinate clinical services to ensure high-quality care for patients in their homes as prescribed by physicians and in compliance with applicable laws and regulations.
- Provide clinical services within the scope of practice, as defined by state laws governing nursing practice, in accordance with the plan of care and in coordination with other members of the healthcare team.
- Manage referrals, ensure appropriate clinician assignments, evaluate patient orders, and plot start of care visits.
- Determine patient home health benefits, medical necessity, and ongoing insurance approvals.
- Continuously assess patient needs and render care that is individualized to their needs, meets home health eligibility criteria, and is in accordance with physician orders.
Requirements
- Current RN licensure in the state of Washington
- Current CPR certification required
- Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation
