Job Description
As a Hospice RN Case Manager, you will play a pivotal role in delivering exceptional end-of-life care to our patients and their loved ones. Your expertise will be valued and respected as you develop trusting relationships and provide empathetic support.
You will assume full nursing responsibility for creating personalized Plans of Care, continuously evaluating your performance, and making adjustments to ensure optimal productivity and quality of care.
- Conduct initial nursing evaluations to determine eligibility for hospice services within 48 hours of referral.
- Identify and re-assess patients' physical, psychosocial, and environmental needs every 15 days.
- Develop and coordinate individualized plans of care while maintaining accurate clinical notes.
- Document patient problems, goals, interventions, and responses to care.
- Collaborate with patients, families, attending physicians, and other interdisciplinary team members to deliver seamless care.
Requirements
To excel in this role, you must possess:
- One year of clinical experience.
Licensure and Certification
Please note that you must meet the following requirements:
- Current RN licensure in the state of practice and one year of clinical experience.
- Current CPR Certification.
- Valid driver's license, vehicle insurance, and access to reliable transportation or public transit.
