Job Summary:
We are seeking a skilled and experienced RN Case Manager to join our team on a travel nursing assignment in Juneau, Alaska.
The ideal candidate will possess a current state license as a RN and have at least 2 years of recent experience as a Case Management RN.
Job Description:
This is a 12-hour day shift position with a 36-hour workweek. The successful candidate will be responsible for providing comprehensive case management services to patients and their families.
Responsibilities:Conduct thorough assessments of patients' medical needs and develop individualized care plans. Coordinate with healthcare providers to ensure seamless patient care transitions. Maintain accurate and up-to-date patient records. Collaborate with interdisciplinary teams to promote patient-centered care.
Requirements:Current state license as a RN. At least 2 years of recent experience as a Case Management RN. Excellent communication and interpersonal skills.
