Astrana Health is seeking a dynamic RN Clinical Supervisor. This position involves overseeing a team of clinical and nonclinical care managers for our Enhanced Care Management (ECM) team. This person will play a pivotal role in providing clinical guidance and expertise for high-quality, patient-centered care. The role combines operational management, clinical oversight, and program development to ensure effective care coordination for individuals with complex medical and behavioral health needs. The RN Clinical Supervisor will optimize the care management process, ensuring the implementation of care plans, staff training, and quality improvement efforts.

Team Management & Development:- Supervise, mentor, develop, and provide clinical oversight to a team of ECM nonclinical/clinical LCMs, including onboarding and training staff
- Monitor daily performance, including case management actions (e.g., assessments, care plans, IDTs), and provide guidance on workflow and documentation standards
- Conduct weekly 1:1 meetings, assess staff performance, and implement performance improvement strategies
- Develop and maintain/update training materials (e.g., job aides, workflows) using tools like PowerPoint, Visio, and Canva
- Lead IDT meetings and generate/present reports on member engagement and KPIs to executive leadership
Clinical Oversight & Consultation:- Provide clinical consultation on ECM Health Risk Assessments and review and sign off of patient care plans
- Collaborate with Care Managers and multidisciplinary teams to ensure appropriate management of care, focusing on acute conditions, chronic diseases, and medication management
- Guide Lead Care Managers in health education, preventive care, and chronic disease management for vulnerable populations
- Collaborate in addressing Social Determinants of Health (SDoH) and enhancing connections to community-based organizations
- Utilize data to evaluate outcomes and adjust care plans accordingly for continuous improvement
Engagement & Coordination:- Assist in ensuring smooth transitions of care from hospitals, behavioral health providers, and primary care physicians to prevent readmissions and ensure post-discharge follow-up
- Collaborate with ECM Specialists on ADT alerts and post-discharge care coordination
Quality Improvement & Compliance:- Participate in ongoing quality improvement efforts, such as case reviews, chart audits, and preventive care activities
- Ensure compliance with regulatory standards and maintain documentation accuracy in line with Managed Care Plans (MCPs)
- Monitor and report on monthly performance metrics, including invoicing and claims submission
- Valid Registered Nurse (RN) license in California
- Experience in managing or supervising clinical staff, including performance management and mentorship
- Strong clinical problem-solving skills and the ability to assess and manage care needs
- Basic computer skills (Microsoft Office Suite, email, data summarization in spreadsheets)
- Experience in case management or Cal Aim programs
- Bachelor of Science in Nursing (BSN)
- Bilingual proficiency in English and Spanish
- Experience with healthcare applications (e.g., EZCap, MCG)
- Proficiency in data analysis and performance tracking.
- Experience working with vulnerable populations, including those with a history of trauma, homelessness, or substance use disorders
- Familiarity with managed care functions and health plan requirements
- This is a hybrid position. The home office is aligned at 1600 Corporate Center Drive, Monterey Park, CA 91754. Travel to community events or clinics will be required as needed.
- The total compensation target pay range for this role is between $100,000 - $125,000 annually. This pay range represents our national target range for this role. Total compensation will be evaluated based on experience, certifications, and skillsets.
