Job Overview:
We are seeking a compassionate, organized, and dedicated Case Manager/Group Facilitator to join our team. This role is a combination of case management and group facilitation, where the incumbent will support individuals through case planning and intervention services while also leading and guiding group sessions. The ideal candidate will have experience in social services, counseling, or a related field and a passion for helping individuals achieve their goals and improve their well-being.
Key Responsibilities:
Case Management:
- Conduct initial assessments to evaluate clients’ needs, strengths, and goals.
- Develop individualized service plans in collaboration with clients to address their needs.
- Monitor client progress, adjusting plans as necessary to ensure positive outcomes.
- Provide resource referrals and assistance in accessing community services, including housing, employment, healthcare, and education.
- Maintain accurate, up-to-date records in compliance with agency policies and regulatory requirements.
- Provide crisis intervention and emotional support as needed.
- Advocate for clients with external service providers, ensuring clients receive the services they need.
Group Facilitation:
- Lead group sessions designed to promote personal growth, skill development, and peer support.
- Develop and implement group curricula and activities that align with client needs and goals.
- Foster a positive and inclusive group environment that encourages participation and mutual respect.
- Ensure that all group sessions are conducted according to established guidelines and in a safe, non-judgmental manner.
- Monitor group dynamics and address any conflicts or challenges within the group setting.
- Provide feedback to clients individually or in a group setting, reinforcing progress and setting future goals.
Qualifications:
- Bachelor’s degree in Social Work, Psychology, Sociology, or related field (Master’s degree preferred).
- years of experience in case management, group facilitation, or a similar role.
- Strong communication and interpersonal skills with the ability to build rapport with clients from diverse backgrounds.
- Knowledge of community resources and ability to make referrals as appropriate.
- Ability to lead and facilitate groups effectively, with an understanding of group dynamics.
- Excellent organizational skills and attention to detail, with the ability to manage multiple clients and tasks.
- Empathy, patience, and a genuine desire to help others succeed.
- Knowledge of confidentiality laws and ethical practices in social services.
- Certification or training in specific therapeutic techniques or group facilitation (e.g., Cognitive Behavioral Therapy, Motivational Interviewing) is a plus.

Pay: $20.00 - $23.00 per hour
