Job Summary
As a Certified Occupational Therapy Assistant with AT Home Care & Hospice, you will play a vital role in providing direct care to patients in their homes, working collaboratively with our interdisciplinary team to ensure optimal patient outcomes.
Essential Duties:
- Conduct ongoing evaluations of patient functional status.
- Develop and implement treatment plans to promote patient independence.
- Provide instruction to family/caregivers on treatment regimens.
- Follow treatment programs as advised by Registered Occupational Therapists.
- Document patient progress and maintain accurate clinical records.
- Ensure compliance with relevant laws and regulations.
- Additional duties as assigned.
