As a Registered Nurse with our team, you will be responsible for assessing patients' needs, developing personalized care plans, and coordinating with the interdisciplinary team to ensure optimal care outcomes. Responsibilities
- Complete comprehensive assessments of patients and families to identify their unique needs and goals
- Develop and implement individualized care plans that address patients' physical, emotional, and social needs
- Collaborate with the interdisciplinary team to coordinate care and ensure seamless transitions between healthcare services
- Administer medications and treatments as prescribed by physicians in accordance with established protocols
- Educate patients and families on self-care techniques, medication management, and disease prevention strategies
- Maintain accurate and timely documentation of patient information and progress
- Participate in ongoing quality improvement initiatives to enhance the quality of care and services provided
Qualifications
- Graduate of an accredited nursing school with current licensure to practice in the state of operation
- 1 year of nursing experience required; 3 years preferred
- Experience in home health or a similar setting preferred
