Job Summary:
We are seeking a dedicated and organized Care Coordinator/Field Supervisor to join our team. This role combines field supervision, case management, and caregiver scheduling to ensure our clients receive the highest level of care. The ideal candidate will have strong communication skills, attention to detail, and a passion for helping others.
Key Responsibilities:
- Conduct home visits to assess client needs, monitor care quality, and ensure client satisfaction.
- Develop, implement, and adjust personalized care plans in collaboration with the Director of Client Services.
- Supervise and support caregivers, providing guidance and addressing performance concerns as needed.
- Coordinate caregiver schedules to meet client needs, ensuring proper coverage and compliance with care plans.
- Act as a liaison between clients, families, and caregivers, addressing concerns and resolving issues promptly.
- Maintain accurate and up-to-date client records, including care plans, visit notes, and communication logs.
- Assist with caregiver training and onboarding, ensuring they understand company standards and client expectations.
- Monitor compliance with company policies, state regulations, and industry best practices.
- Participate in team meetings and provide input on ways to improve client care and operational efficiency.
Qualifications:
- Previous experience in home care, case management, or a similar field preferred.
- Strong organizational and time-management skills.
- Excellent communication and interpersonal skills.
- Ability to handle sensitive information with discretion and maintain confidentiality.
- Proficient in basic computer software and scheduling tools.
- Reliable transportation and a valid driver’s license.
