Responsibilities
- Evaluate and assess patient needs to determine the level and frequency of care required
- Develop and implement patient-specific care plans using available tools and resources
- Communicate effectively with the care team, physician, payors, patients, and families to address care needs and fulfill the patient's care plan
- Deliver high-quality skilled care and supervise other care team members as needed
- Document care delivery daily in our EMR system
