Your primary responsibilities will include evaluating and assessing patients to determine their appropriateness for care, developing patient-specific care plans, and communicating with the care team and other stakeholders to ensure seamless care coordination. You will also deliver high-quality skilled care and supervise the care of other team members. In addition, you will thoroughly document care delivery daily in our electronic medical record (EMR) system.
Requirements
To be successful in this role, you must possess a strong foundation in nursing principles and practices, as well as excellent communication and interpersonal skills. You should also be able to work independently and as part of a team, with a passion for delivering high-quality patient care. A current RN license and relevant experience are required for this position.
