Description
The Intake Coordinator is responsible for the screening and recertification of all referrals. Ensures coordination of patient care between various agency departments occurs in a timely manner and is responsible for ensuring all clients receiving services have employees scheduled to provide those services. Matches clients’ requested shifts with caregivers’ personalities and skills. This position requires an LPN license and reports to the Director of Patient Care Services.
Pay: $22-$25/Hour DOE
The following duties are normal for this position. Other duties may be required and assigned.

Act as the primary point of contact for prospective patients and their families, providing information about services and admission procedures.

Build and maintain positive relationships with healthcare professionals, physicians, and referral sources in the community.

Address inquiries, concerns, and requests from patients and families in a prompt and compassionate manner.

Assists with maintaining a complete and accurate visit schedule, tracking staff and availability, cancellations, late arrivals and/or early departures from assigned shifts, fully utilizing assigned scheduling software.

Monitors electronic clock-ins and clock-outs for field staff, addresses missed clocks, and troubleshoots as needed.

Finds replacements for staff that call in sick or are unable to work as needed.

Recruits and assists in the hiring process and orientation process when warranted.

Verify Medicaid/Medicare and commercial insurance benefits, this includes calling insurance companies to check benefits and using state portals.

Obtain accurate referral information.

Ensure physician orders are communicated with accuracy, thoroughness, and understanding.

Enter admissions into systems.

Obtain authorization and re-authorizations for patients that have started care.

Work on reports when assigned.

Participate in team meetings.

Participate in the information management and performance improvement programs, as needed.

Completes annual education requirements.

Always maintains and ensures patient confidentiality.

Maintains confidentiality of all records and reports maintained for all patients.
