At Home Quality Care seeks a self-motivated leader and team-oriented Director of Nursing for our TINLEY PARK, IL branch. The ideal candidate will provide leadership and support to our clinical staff while ensuring the delivery of high-quality patient care in accordance with the company’s objectives and policies, through planning, coordination, and implementation.
Responsibilities :
- Develop and implement care plans in collaboration with physicians and healthcare team.
- Oversee nursing administration, including staffing, training, and performance evaluations.
- Ensure compliance with all regulatory requirements, including State, Federal & CMS guidelines.
- Coordinate with other departments to provide comprehensive patient care.
- Coordinate, and schedule, the daily workload of the agency, utilizing company personnel to their fullest capacity.
- Provide daily supervision of field nurses, LPNs, therapists, and Home Health Aides, to ensure the quality of services and maintenance of standards.
- Provide teaching and counseling necessary to enhance the delivery of services, and the professional development, of nursing and aide staff.
- Identify, and evaluate, patient needs and appropriateness of home care, by reviewing the initial MD orders before assignment of direct care staff.
- Assist with the orientation of new clinical staff, to familiarize them with company policies and procedures.
- Ensure that all patient documentation and files are ready for coding and billing
- See patients in the home setting, as needed.
- Participate in CHAP accreditation audits, interdisciplinary team conferences and represent Agency, in the community, as requested.
- Ensure that the agency is CHAP and State survey ready.
- Perform Quality Assurance (QA) for all patient charts.
- Review each nurse's schedule for recertification, discharges, and supervisory visits.
- Review weekly schedule submitted by nurses for changes or updates.
- Perform other duties as directed, by the Administrator, Regional Director of Clincal Operations, etc.
