Job Summary
We are seeking a dedicated and compassionate Service Coordinator to join our team. The ideal candidate will play a crucial role in supporting the elderly and individuals with disabilities by coordinating services and ensuring they receive the necessary support to thrive. This position requires strong organizational skills, effective communication, and a passion for making a positive impact in the lives of others.
Duties
- Coordinate and manage services for individuals, ensuring their needs are met effectively.
- Develop and implement individualized service plans in collaboration with clients and families.
- Maintain accurate records and documentation related to client services, progress, and outcomes.
- Monitor EVV's and digital timesheets
- Build and maintain positive relationships with clients, families and case managers
- Providing guidance and support to our HHA's as needed.
- Conduct assessments to identify client needs and recommend appropriate services or interventions.
Experience
- Proven skills in relationship management with clients, families, and community partners.
- Informed about the Long Term Care program
- Strong supervisory skills to effectively manage staff involved in service coordination.
- Excellent organizational skills with the ability to manage multiple tasks simultaneously.
- Strong interpersonal skills to foster positive relationships within the community.
