POSITION SUMMARY:
The position provides comprehensive, patient-centered care across various clinical settings, ensuring our community's highest standard of medical service. The Nurse Practitioner will assess, diagnose, treat, and manage both acute and chronic health conditions, working closely with physicians, specialists, and other healthcare providers to deliver quality care. CORE FUNCTIONS:
- Conduct thorough patient assessments, including taking medical histories, performing physical exams, and ordering and interpreting diagnostic tests.
- Diagnose and manage a wide range of health conditions, both acute and chronic, and prescribe medications as appropriate.
- Provide patient education on disease prevention, health promotion, and the management of medical conditions.
- Collaborate with physicians, specialists, and interdisciplinary teams to develop and implement individualized care plans.
- Monitor patient progress, adjust treatment plans as necessary, and ensure appropriate follow-up care.
- Document patient interactions, assessments, and treatments in compliance with hospital policies and state regulations.
- Participate in patient care coordination, including referrals to specialists and follow-up appointments.
- Engage in continuous professional development, staying current on advancements in healthcare, clinical best practices, and healthcare regulations.
Must-Have Qualifications:

Pay: $85,000.00 - $95,000.00 per year
