Job Summary
The Almost Home Coordinator position is responsible for providing holistic case management to residents (six families) in the Almost Home shelter program and all related responsibilities, including supervision of shelter monitors.
Position: Full time salaried position with health benefits, retirement
100% Almost Home (75% case management activities, 15% Continuum of Care activity, 10% HMIS and documentation)
Flexible Schedule: Work hours are 8 hours per day, with standard hours scheduled between 8 AM and 6 PM. While we make every effort to honor these hours, some evenings and weekends may be required based on the needs of the role. Flexibility is appreciated when necessary.
Minimum Qualifications: Bachelor’s or Master’s degree in Social Work (MSW), Human Services, Psychology, or a related field, or equivalent combination of education and relevant experience in case management or social services.
Supervision: Responsible to Program Director
Specific Job Duties:
- Screen potential residents and conduct intake processes for new clients; same day intake when space allows.
- Determine eligibility and duration of stay (up to 45 days) in collaboration with the client, based upon the specific issues and needs of each family.
- Develop a housing plan in conjunction with client and support client in exploring potential housing; when eligible, work with Going Home Coordinator to ensure a seamless transition into Rapid Re-Housing as quickly as is practical.
- Identify and make appropriate referrals to resources and services which address needs of clients; work with the service community to ensure seamless service delivery within the continuum of care.
- Enroll all AAHS residents in relevant programs; ensure best possible employment or education goals for each client; provide ongoing services to all shelter residents as needed according to policies and procedures.
- Work with clients and daycare centers to provide child care for children; maintain records of child care vouchers.
- Work with UGA or other interns assigned to case management to provide a positive learning experience and maximize services to residents.
- Review housing plans for each resident at weekly case management meetings; conduct housing focused case management for residents.
- Maintain current and accurate files on each client, utilizing professional standards of record keeping and confidentiality.
- Maintain HMIS database with accurate and updated client information.
- Supervise shelter monitors and cover shifts when vacancies arise; maintain effective communication with all staff ensuring that the specific needs of residents are communicated fully to night/weekend staff.
- Attend required case review, staff meetings and any other meetings as instructed by ED.
- Troubleshoot problems that arise related to shelter function; work with the Program Director to establish and execute solutions.
Skills Required:
- Excellent organizational skills, superior oral and written communication skills.
- Flexibility and a team oriented attitude; willingness to work outside conventional work hours.
- Ability to use empathic, professional judgment to best serve the interests of the residents in case management decisions. AAHS utilizes the following concepts in effective case management and planning: self-determination, trauma informed care, motivational interviewing, critical time intervention, harm reduction, progressive engagement.
- Counseling and/or experience with the homeless population is preferred.
- Case Manager must have a valid GA driver’s license, and reliable transportation.
