Job Summary:
The Community Health Worker works closely with primary care providers and/ or addiction medicine providers to facilitate referrals for counseling and provides case management, assisting patients to identify and access the help they need, and document encounters in the electronic medical record ensuring that all pertinent information including; referrals, release of information, consents and other pertinent documentation is entered appropriately. Cross-training between primary care provider locations: Tully Family, Athol Hospital, & Athol ED.
Responsibilities:
- Provide medical, behavioral and community based care coordination to population served
- Coach patients in effective management of their chronic health conditions and self-care.
- Assist patients in understanding care plans and instructions. Provide culturally and language appropriate services.
- Help patients in properly accessing and utilizing resources including scheduling appointments, assisting patients with completing applications and registration forms.
- Assist patients in accessing health related services and overcoming barriers to obtaining needed medical care and social services.
- Facilitate communication between patients, services and providers. Encourage patients to be active, engaged participants in their health.
- Help patients connect with transportation resources and give appointment reminders as needed.
- Conduct comprehensive screenings and assessments activities including mental health status, general health screenings, and social determinants of health.
- Assist patients in accessing tele behavioral counseling services as well as off-site community based clinicians and non-clinical resources as needed.
- Manage data and appointments in the hospital’s electronic medical record.
- Obtain insurance authorizations/referrals for coordinated services (if applicable)
- Implement patient-centered case management for each patient.
- Provide ongoing follow-up, motivational interviewing and goal setting with patients.
- Continuously expand knowledge and understanding of community resources and services and create a repository of this information for healthcare system use.
Collaborate with caregivers as a member of the patient’s care team and as patient advocate to discuss patient’s needs/services.
- Help patients navigate services throughout the healthcare system and to understand their health insurance coverage and responsibilities.
- Participate in weekly supervision and program huddles/staff meetings.
- Track and submit data; complete monthly reports.
- Manage time and resources in a skilled manner.
- Demonstrate appropriate boundaries in all interactions.
- Attend relevant trainings, conferences, and community events.
- Follow Heywood Healthcare’s policies and procedures.
- Maintain confidential information.
- Additional duties will be at the discretion of the department manager.
- Associate degree and/or equivalent combination of experience and education, CHW certification preferred. CHW certification must be completed within 6 months of the hire date. Funding provided for CHW certification requires 1 year of fulfillment of the RCORP role.
