Job Summary : We are seeking a highly skilled and compassionate Dual Licensed Psychiatric Mental Health Nurse Practitioner (PMHNP) and Family Nurse Practitioner (FNP) to join our team. This role will involve providing psychiatric consults in a rehab setting and managing chronic care for residents in our senior living community. The ideal candidate will have a strong background in both psychiatric and family health, excellent communication skills, and a passion for improving patient outcomes.
Key Responsibilities :
- Conduct comprehensive psychiatric evaluations and provide ongoing psychiatric care and management for patients in a mental health authority setting via telehealth four (4) days per week.
- Conduct chronic care management and behavioral health management for residents in a senior living community, focusing on the management of chronic diseases such as diabetes, hypertension, and heart disease in-person one (1) day per week in the Baytown, TX area.
- Develop and implement individualized treatment plans for patients with mental health and substance use disorders.
- Provide medication management.
- Collaborate with a multidisciplinary team, including physicians, therapists, and social workers, to ensure coordinated and comprehensive care.
- Perform routine health assessments, diagnose and treat common health conditions, and provide preventive care.
- Educate patients and their families about chronic disease management and healthy lifestyle choices.
- Maintain accurate and up-to-date medical records in compliance with regulatory standards and company policies.
