Job Overview
We are seeking a dedicated, self-motivated and passionate Community Liaison to serve as a bridge between ATL Hospice and Palliative Care our community. This role is essential in accomplishment of our Mission to improve the quality of life and provide comfort to patients and their families through our tailored comprehensive end of life care. The ideal candidate will possess strong communication skills, and the ability to promote the ATL Hospice and Palliative Care Vision to become a trusted member of the community through our compassionate service and education.
Responsibilities
- Conduct initial meeting with the patient and family providing an overview of ATL Hospice and Palliative Care services and obtain consents for care.
- Focus on developing strong relationships with community stakeholders such as healthcare providers, assisted living facilities and other referral sources.
- Provide timely and effective communication with referral sources and potential patients, by making regular and planned calls to secure new referrals.
- Convert referrals to meet and exceed current census goals.
- Assist in implementing marketing and outreach strategies to promote ATL Hospice and Palliative Care Services, and to build the brand as the preferred Hospice and Palliative Care provider in our service area.
- Educate the community about available resources and programs related to ATL Hospice and Palliative Care.
- Deliver presentations and engage in public speaking to raise awareness about Hospice Care, eligibility criteria and benefits.
- Maintain confidentiality and respect patient privacy in all communications and activities.
Qualifications
- Experience in healthcare related field such as long term care is highly desirable.
- Previous marketing and admissions experience
- Basic computer skills to include use of Adobe.
- Excellent public speaking skills with experience presenting to diverse audiences.
- Strong organizational skills.
- Ability to work collaboratively with intake coordinator as well as the leadership team while also being self-motivated.
