What are the responsibilities and job description for the Population Health RN position at Atlantic General Hospital?
Performs standard organizational duties for hospital’s Population Health Department including clinical, educational and administrative duties. Able to function independently to provide care at vaccine clinics, hypertension clinics, First Aid care stations, and health fairs in the community. Performs primary care nursing on wellness projects. Works closely with the Director Community Health in coordination of databases, community events, mailings, and projects. Experience with vaccine databases, Microsoft Word and Excel is desirable.
Current licensure as a Registered Nurse or similar clinical credentials.
- Bachelor’s Degree Nursing
- American Heart Association, Health Care Provider CPR certification required.
- Current appropriate state driver’s license required.
- Knowledge of Microsoft Word, Excel and Access programs.
Three years of clinical experience preferred.
Ability to convey information clearly in English both verbal and written.
- Ability to organize and manage time effectively, setting appropriate priorities.
- Ability to make independent decisions consistent with policies, procedures and hospital standards.
- Ability to understand database driven information with appropriate training.
- Flexible scheduling to include evenings and weekends as needed.
Salary Range: $28.00/hour to $46.20/hour (credit given for relevant experience)
