We are actively searching for aRNCase Manager. This position provides complex assessment and discharge planning functions for patients located in the hospital setting. Utilizes behavioral interviewing techniques to identify patients at risks for ED and Inpatient recidivism, and develops strategies to prevent readmission. Utilizes conceptual framework from Social sciences to develop interventions which promote optimal patient outcomes.
Qualifications:The ideal candidate will have a current RN license (with a BSN or related Bachelor's Degree preferred). Candidate must have knowledge of community based resources and prior Case Management experience is preferred. Must have the ability to multi-task and perform functions in a fast-paced setting. Knowledge of insurance and associated coordination of healthcare services is required.
