Job Summary As a Chronic Care Manag ement Patient Care Coordinator at Atlas Medical Care, you will play an essential role in providing exceptional home health and end-of-life care to our patients. Based in our Mesa office, you will serve as an integral support system for both our patients and providers. This position is not patient facing, as we do not see patients at our m ain office . Your responsibilities will include the meticulous collection of vital health data for seamless entry into our electronic medical record (EMR) system, as well as the efficient coordination of schedules among residents and providers. Strong communication skills and attention to detail are vital, as you will keep patients, families, powers of attorney, and healthcare providers informed and engaged through phone calls and messaging. You will also assist patients in developing patient-centered goals and care plans while receiving feedback from providers on necessary adjustments. Additionally, you will ensure strict compliance with all regulatory standards.
