We are seeking an experienced and dedicated Community Health Social Work Program Director to lead our client’s social work initiatives and oversee community-based health programs. The Program Director will be responsible for program development, staff supervision, clinical oversight, and ensuring compliance with state and federal regulations. This leadership role requires an LCSW and a strong background in community health, behavioral health, and social services.
- Key Responsibilities :
- Oversee the design, implementation, and evaluation of community health and social work programs.
- Develop strategic initiatives to improve access to care, patient outcomes, and community engagement.
- Ensure that programs meet local, state, and federal regulations and align with organizational goals.
- Identify gaps in services and implement innovative solutions to improve community health outcomes.
- Provide clinical supervision and support to licensed and unlicensed social work staff.
- Ensure the delivery of high-quality, evidence-based behavioral health and social services.
- Monitor case management, crisis intervention, and referral services for at-risk populations.
- Uphold compliance with HIPAA, Medicaid, Medicare, and other relevant guidelines.
- Recruit, train, and mentor a team of social workers, case managers, and support staff.
- Conduct performance evaluations, professional development, and training programs.
- Foster a collaborative and inclusive work environment that promotes staff well-being.
- Establish and maintain relationships with local agencies, healthcare providers, and community organizations.
- Advocate for policy changes and resources to address social determinants of health.
- Lead outreach initiatives to engage underserved communities and improve service utilization.
- Requirements :
- Licensed Clinical Social Worker (LCSW) required in New York.
- Master’s degree in Social Work (MSW) from an accredited institution.
- Minimum 3 years of experience in community health, social work, or a related field.
- Minimum 2 years of supervisory / management experience preferred.
- Strong knowledge of Medicaid, Medicare, and social service funding streams.
- Experience in grant writing, budgeting, and program evaluation is a plus.
- Excellent leadership, problem-solving, and communication skills.
