We are seeking a dedicated and compassionate Palliative Care Nurse Practitioner (NP) to join our client’s interdisciplinary team, providing holistic care to patients with serious or life-limiting illnesses. The ideal candidate will have experience in palliative care or hospice, demonstrating expertise in symptom management, emotional support, and end-of-life care planning. This role involves working collaboratively with physicians, nurses, social workers, and other team members to ensure the highest quality of patient-centered care.
- Key Responsibilities :
- Perform comprehensive assessments of patients with chronic, advanced, or terminal illnesses, including physical, emotional, social, and spiritual needs.
- Develop and implement individualized care plans focused on symptom management, quality of life, and patient goals.
- Provide pain and symptom management, including prescribing medications and treatments in accordance with state regulations.
- Facilitate discussions about advance care planning, goals of care, and end-of-life decisions with patients and families.
- Collaborate with the interdisciplinary palliative care team to ensure a coordinated approach to patient care.
- Serve as a resource and consultant to other healthcare providers regarding palliative care principles and practices.
- Provide education and emotional support to patients' families and caregivers, helping them navigate the complexities of serious illness and end-of-life care.
- Act as a liaison between patients, families, and the healthcare team to address concerns and ensure understanding of care plans.
- Educate patients, families, and staff on palliative care principles, including symptom management and advance care planning.
- Advocate for patients' needs and preferences within the healthcare system.
- Requirements :
- Master’s degree in Nursing with certification as a Nurse Practitioner (NP).
- Current NP licensure in New York.
- Minimum of 1 year of experience in palliative care or hospice.
- Spanish-speaking abilities preferred but not required.
- Excellent communication and interpersonal skills, with the ability to have sensitive conversations about serious illness and end-of-life care.
- Strong clinical judgment and critical thinking skills.
- Ability to work collaboratively within a team and independently when needed.
- Empathy, compassion, and a commitment to patient-centered care.
Please note - The annual base salary range is $130k to $165k. Actual compensation offered to the successful candidate may vary from posted hiring range based upon geographic location, work experience, education, and / or skill level, among other things. Details about eligibility for bonus compensation (if applicable) will be finalized at the time of offer._
