Must Have Prior RN Homecare Experience

Functions of Registered Nurse (RN):
- Provides skilled hands on care to patients in the comfort of their own homes
- Develop the patient's plan of care for the safe and effective delivery of services in their homes
- Work closely with an interdisciplinary team to achieve optimal patient outcomes
- Provide education to empower patients to manage their chronic disease processes

Requirements
Current State of Connecticut Registered Nurse License
At least one (1) year of RN Homecare experience
Current driver's license
Reliable transportation
Strong clinical assessment, organizational, and communication skills
Ability to work independently and as part of a team.

Pay: $78,000.00 - $95,000.00 per year
